After testing pg_query_params, I found that it supports reusing parameters. In other words, the script below works as expected (no error message). This was IMO not clear from the documentation.

Test script:

pg_query_params(
    'SELECT * FROM tname WHERE foo = $1 OR bar = $1',
    array('value'));

This bug has been fixed in the documentation's XML sources. Since the
online and downloadable versions of the documentation need some time
to get updated, we would like to ask you to be a bit patient.
