Our applications work in Unicode. So, everything we retrieve from the database has to be encoded to Unicode or Unicode from source. That's why we need to identify nvarchar fields (ntext also) and bypass the encoding with them.
mssql_field_type only returns 'char'.
Maybe a new function that returns the original data type (without making an inner query, if that's possible) could be the best solution.

If MSSQL doesn't differ them, obviously PHP can't do it too.
