$date = date("m/d/y &  h:i A T" , $timestamp);

I have tested this thoroughly.  The T argument causes PHP to encounter an "Access Violation" which subsequently crashes IIS and then win2k.

This seems to be fixed in the latest CVS and I think it was also fixed in 4.0.5. You can download PHP 4.0.5 from If you still encounter this problem then please open a new bug report.
