1. phpinfo() shows that mysql installed

2. file having problems with is createdb.phtml:

4. Any suggestions on what is wrong. Is there another way to setup embedded mysql with a database?

Some text lines are wrapped in email, but not wrapped in files. Mysql is shown as enabled via phpinfo():

I also tried using my real hostname and real username on the network which my laptop (which has PHP4.0.4
installed) is connected, but it made no difference. I think my problem is that I do not know how to access mysql administration to do such things as create/modify grant tables, create users, setup passwords, etc. I didn't see any readme files for Mysql3.23.22-beta that is in PHP4.0.4? 

It seems that there is plenty of help on administration of mysql for MySQL installations that are unbundled with PHP,
but I could not find readme's which addressed embedded MySQL with PHP. I also set mysql.defaults and it didn't appear to help either.

You need to have mysql server running.
The bundled mysql is only a client.
