PHP 4.0.4 fails to let me in with the Basic Auth example from the manual.

Can you please provide any other information possible. Where you got your binaries from, are you using apache mod or CGI? We can then ask members of the QA team to check this out and verify the bug. Im not even 100% sure if it should work in your setup.. Ill have to check that myself but it might be a windows thing (tm), has it worked with previous versions of PHP?

Here we go:
Compiled myself. 
DSO == apache mod.
It had worked quite a few ago, when (IIRC) Shane Caraveo made it possible to build win32 Apache module (which was... spring this year, don't remember exactly).
Back then, I checked it out only because I wanted to know if it'd work. It did.
I haven't used it since then, until tonight, so I can't say when it got broken.

works for me just fine with PHP 4.0.5.
