System Description:
Dual 450MHz Pentium II
2 - 18GB SCSI HDD
1 - 75GB Ultra 100 HDD
1GB of RAM
Guillemot Cougar Video Card 32 MB TV in/out

I've noticed that this happens more often when using the sample user error handler.
The system will not produce this error until the user error handler is called.  Once it is called, the error is produced every time any .php page is accessed.

.html files work fine,
.php files produce this error.

Please try php 4.0.6 and reopen if this problem still persists. There is a big chance this is fixed.
