This bug has been fixed in SVN.

In case this was a documentation problem, the fix will show up at the
end of next Sunday (CET) on pecl.php.net.

In case this was a pecl.php.net website problem, the change will show
up on the website in short time.

sorry, I didn't properly filled out the bug submission, 
hence your misunderstanding:
the original bug was that if you filled out the form, but 
your account was already taken, the resulting page was 
missing both the form and the error message, it was just a 
broken html page.
I fixed this bug, so now if you do that (try to register 
with an existing username or email), you will get the 
resulting page properly rendered and an error displayed.

for resolving your issue: you should wait for your account 
to get either approved or rejected, then proceed with your 
account or re-apply if needed.

thanks for you response,
my previous application was submit more than a month ago,

now I have discussed the topic of adding a PECL extension on 
the pecl-dev@lists.php.net mailing list,

but can't re-apply agian,

so who can I refer to for help? thanks.
