First of all, thanks for this great extension.

While trying to develop an COM/ADO driver for SQL Server I've found three small bugs:

- The name parameter PDO::lastInsertId was causing an error when it was omitted (it's optional).

- Last version was not compiling in Windows (variable definition).

- In php_pdo_user_fetch_error_func, the description was being copied from the position 0 instead of 1.
