apc.include_once_override is not documented.

Reproduce code:

This setting was was introduced as default in APC xx.11 now becomes configurable but it is not clear what happens when you set it.

Expected result:

The INSTALL doc should explain what happens when you use it and what the implications are.

Actual result:

Unknown what apc.include_once_override should be set to.

Right now, if you enable the flag, expect segfaults :).

I think I'll wait till the code is "complete" before I update docs.

The apc_opflags checks required to make apc.include_once_override = on; safe and the documentation for the override.
