==> This source is Okay:

session_start();
$strStream  = "<html><head>...";
$strStream .=
"<img src=\"MYIMAGE.JPG\">";
echo $strStream;

=======================
==> dont so this source:

session_start();
$strStream  = "<html><head>...";
$strStream .=
"<img src=\"\">";
echo $strStream;

PROBLEM:
In the second source, the image src is empty.
Look now in your Webservers directory for save
