I have created a small patch that changes Zend/zend_highlight.c to include proper DOCTYPE, <html>, body and other page composition tags.

I also changed from the deprecated <font> to <span> and CSS

I currently use this on my local gentoo development server and have provided a patch at

apply patch in the top level source directory

I think the idea is that you can use it in a standalone document if you want to, or as part of another file. This is why it doesn't send the headers.
