The example code in chapter 17 HTTP Authentication with PHP is:
echo "<p>You entered {$_SERVER['$PHP_AUTH_PW']} as your password.</p>";

This doesn't work as the key should be 'PHP_AUTH_PW' and not $PHP_AUTH_PW

The correct code is:
echo "<p>You entered {$_SERVER['PHP_AUTH_PW']} as your password.</p>";

This bug has been fixed in CVS. You can grab a snapshot of the
CVS version at http://snaps.php.net/. In case this was a documentation 
problem, the fix will show up soon at http://www.php.net/manual/.
