Any page when php-4.0.3pl1 is loaded (DSO) will cause Apache to segfault.  If PHP-4 is not loaded, Apache runs normally.

Try configuring using --with-mysql=/path/to/mysql
and recompile. Do this for both php3 and php4.

This has propably been the bug with running mod_perl together
with PHP and it has been fixed in CVS. So please try latest snapshot
from http://snaps.php.net/  or wait for PHP 4.0.4 to be released.
