mb_convert_encoding doesnt't properly handle the value of mb_substitute_character when the third argument is different from the value of mb_internal_encoding.

Test script:
---------------
mb_substitute_character(0xFFFD);
var_dump(mb_convert_encoding("\x80", "Shift_JIS", "EUC-JP"));

Expected result:
----------------
string(1) "?"

Actual result:
--------------
string(0) ""
