When PHP4ISAPI.DLL is added to the list of ISAPI filters, user name/password authentication always fails.  When it is removed from the list of filters but kept as an application mapping, authentication appears to be disabled for the whole service, allowing any user to access protected pages.  When the DLL is removed entirely, authentication returns to normal.
