When rendering DocBook with tables, PhD warns that there is no handler for tr, th, and td tags.

Easy fix:  add these three elements to the private property $myelementmap in the Package_Generic_XHTML class:

        'tr'                    => 'format_row',
        'th'                    => 'format_th',
        'td'                    => 'format_th',

This bug has been fixed in SVN.
