./configure [...] --with-db2=/usr/BerkeleyDB

The configure script finds BerkeleyDB and adds -ldb to the compile-time options, but according to debug.log, it does not add -L/usr/BerkeleyDB/lib or -I/usr/BerkeleyDB/include.

Please verify that it?s still happening using the latest version of PHP (release 4.0.1pl2 or CVS).
