When I use the search functions on www.php.net to search for
"safe" or "safe mode", none of the matches I get back are in the
main body of documentation; they're all errata or change lists,
or false matches for things like "thread safe".
Is safe mode really undocumented?  Or is your site index messed up?

It is mostly undocumented although there is a section on it in the PHP2 docs.
