I have installed PHP 4.2.2, phpinfo() shows well, also the extensions PHP_MSSQL seems OK. 
Now then I would connect from a PHP-Script to the MSSQL-Database (DB is on a other server and goes well) following  Warning comes: 

MS SQL: Unable to connect to server: DBName in D:\Inetpub\Project1\test_sql.php on line 2

I have installed also MDAC 2.7 and Service Pack 3, the Server is an Windows 2000 English.

With a ASP-Site I can connect to the MSSQL-DB. 

Have anybody a idea?
