With 4.0.6 latest cvs (not 4.0.5; is there a reason the latest cvs option in the versions menu is dated March?), __FILE__ is returning the filename with no path delimiters. For example, if the script is /var/www/foo.php, __FILE__ contains 'varwwwfoo.php'.

Should be fixed in latest CVS.
