I downloaded php-4.02 from php.net and tried do an install as documented in  the install instructions.  I provided the following configure line -

 ./configure --with-mysql=/usr/include/mysql --with-apxs

configure reports the following error -

[root@crosis php-4.0.2]#  ./configure --with-mysql=/usr/include/mysql --with-apxs

Doing an ls of /usr/include/mysql shows - 

[root@crosis php-4.0.2]# ls /usr/include/mysql
chardefs.h*  m_ctype.h*    my_net.h*      mysql_version.h*  sslopt-longopts.h*
dbug.h*      m_string.h*   my_pthread.h*  mysqld_error.h*   sslopt-usage.h*
errmsg.h*    my_config.h*  my_sys.h*      raid.h*           sslopt-vars.h*
history.h*   my_global.h*  mysql.h*       readline.h*       tilde.h*
keymaps.h*   my_list.h*    mysql_com.h*   sslopt-case.h*


Bash show up as -

[root@crosis php-4.0.2]# bash -version
bash -version
GNU bash, version 2.04.3(1)-release (i586-mandrake-linux-gnu)


[rsanders@crosis rsanders]$ uname -a
Linux crosis.engr.sgi.com 2.2.15-4mdk #1 Wed May 10 15:31:30 CEST 2000 i686 unknown

Is there any other information I can provide?

Don't point it at the header file dir.

Try --with-mysql=/usr

PHP needs to see something like
--with-mysql=/usr/local
when mysql is installed as:

/usr/local/mysql/include
/usr/local/mysql/lib

The way you're putting the path, php is try to find the mysql headers in /usr/include/mysql/mysql, or something like it.

Using --with-mysql=/usr

allowed the configure to complete with no further problems.
