Defining --with-gdbm=shared,/opt couses configure to search for gdbm.h in /opt/local/include instead of /opt/include

Try this: 

--enable-dba=shared --with-gdbm=/opt 

instead. Please report back if this helped or not.

No feedback. Update to PHP4.0.3pl1 and reopen this bug report
if problem still exists with it.
