I've tried recompiling, reconfiguring, rebuilding apache, rebuilding mysql, and redownloading the tar.gz (on the off chance I got a dirty d/l) I've even tried compiling without MySQL support, with the included mysql support, and without gd. Apache is 1.3.12, MySQL is 3.22.32. Apache is built with SHARED_CORE and mod_so, apxs apparently works (although this is the first/only shared module I am adding). Here is my apache configure line (apaci):
/usr/src/apache/configure --prefix=/usr/local/apache --bindir=/usr/local/bin --sbindir=/usr/local/sbin --mandir=/usr/local/man --htdocsdir=/usr/local/webdata --enable-rule=SHARED_CORE --
server-uid=xxxxx --server-gid=xxxxx --enable-module=so --enable-module=expires

Thanks!

Hi,

Still have the same problem after upgrading to glibc 2.1.3 on the advice of someone form the php-dev mailing list.

So now I've upgraded to the following:
gcc 2.95.2
make 3.79.1
glibc 2.1.3

My next step will be to build linux-2.4.0-test4, however I cannot do that yet.

Thanks for a great product so far!

Check your /usr/include symlinks.  ie.
cd /usr/include
/usr/include> ls -ld asm linux
lrwxrwxrwx    1 root     root           24 Mar  2 16:05 asm -> ../src/linux/include/asm/
lrwxrwxrwx    1 root     root           26 Mar  2 16:05 linux -> ../src/linux/include/linux/

Are they pointing to your test4 kernel source tree?
