I just loaded php4 on my Windows 2000 server last night as an ISAPI module. Everything was running great last night till this morning. None of my netscape users or customers could get to their websites that were password protected. The authenticate window would popup and the user would type in the user name and password, but they would be denied access. They were not even trying to access a PHP file, just static HTML files.  I tried everything, rebooting the server, different versions of netscape, offsite locations, nothing helped. I then removed the PHP4 ISAPI module. Immediately everything starting working again.
 
Used default PHP.INI settings

Other, similiar bugs have been corrected in recent releases.  Please try a recent release and submit a new bug report if you encounter the problem again.
