Currently the TOC is cached forever and likely it should be removed. The 
following comment exists within the code:

/* TODO:
 * Maybe this isn't worth it.. but this, in theory, allows you
 * to easily add new pages without needing to rebuild the entire
 * section.
 */

Perhaps having it optional is fine but it can be annoying to build the 
manual and wonder why the TOC is wrong.

This report is in reference to the phpweb theme, specifically the 
header() definition inside phd/themes/php/phpweb.php

This bug has been fixed in SVN. Since the websites are not directly
updated from the SVN server, the fix might need some time to spread
across the globe to all mirror sites, including PHP.net itself.
