The PHP documentation contains information about the MCAL extension. In particular it states that MCAL is available from PECL. It isn't.

MCAL is dead and all references to MCAL should be removed from the documentation.

It is available as a PECL extension, see http://cvs.php.net/viewvc.cgi/pecl/mcal/.
Since there have been no PECL releases of the extension it however isn't in the "package list" on pecl.php.net.

You are right though, that extension is dead.

The MCAL docs were recently removed from the PHP manual CVS, and will not show up starting sometime in the near future. Thank you for the report :)
