After trying every suggestion I could find on getting php5 to work with apache2, I finally came across the following thread:

"Apache and PHP - A Fast, Reliable, and Proven Setup"
http://www.apachelounge.com/forum/viewtopic.php?t=570

This setup procedure worked as advertised without any problems. It involved downloading a different php5apache2.dll, installing a microsoft vc5 redistributable and configuring a manifest file.

Since this topic comes up often, it would be worthwile to investigate the differences between this apache module and the module that comes with php5.

You will see that this is stil an outstanding bug and not a documentation or user issue.
