My GD Header files (located in /usr/include/gd/) where not found by the configure script. I had to change:
  ./ext/gd/gd.c, ./ext/gd/gdttf.c ./ext/gd/php_gd.h to point to the gd subdirectory of /usr/include.

If you use "--with-gd=/usr", PHP will find the include files automatically.
