It looks like this is very similar to Bug id #1615.

A file-upload handling script that worked under previous (3.0) versions of PHP no longer works under PHP 3.0.11

Following the documentation at http://www.php.net/manual/features.file-upload.php3 the variable "$userfile" is actually getting set wrong.

It ends up comtaining the original name of the file on the sender's system, rather than the temporary filename in which the uploaded file was stored on the server machine.
