In the Parameters section in http://www.php.net/manual/en/function.mcrypt-create-iv.php, there is a part of the "source" parameter description in the "size" description.

Also, the last item listed in the "See Also" section (Chapter 9.3 of Applied Cryptography by Schneier (ISBN 0-471-11709-9)) should be replaced with "Chapter 9.3 of &book.applied.cryptography;", because it is a global entity

This bug has been fixed in the documentation's XML sources. Since the
online and downloadable versions of the documentation need some time
to get updated, we would like to ask you to be a bit patient.

I left it out initially because I felt it was self-explanatory, but I'm
not bothered one way or the other. I've omitted the bit in brackets
about the default value, though, since it's already noted in the
function description.
