When indexing/building the TOC, livedocs will ignore the title if it is in an unexpected, yet legal, place.

works: peardoc/en/chapters/standards.xml
fails: peardoc/en/chapters/installation.xml

LMK if you want more detials.

Livedocs is no longer in development, the current build system is
titled PhD.

Note: This is not a problem in PhD.
