I'm not sure if this relates only to IIS - I upgraded to 4.2.1 and fileupload didn't work at all so then I used 4.2.3. It's better but still file upload seems buggy. The simplest test I could create is the script below, which generates an endless number of php$HEXNUMBERINC.tmp in the upload temp directory. Each file is about 4.999 KB in size. The only way I could find to stop it is to stop IIS.
