The first notes section tells something about "a" and "a+" opened files.
The second notes section is again telling this, but in a smaller scope. The second note section is only talking about "streams" specifically. 

It's a bit unclear, but not a big problem.

This bug has been fixed in the documentation's XML sources. Since the
online and downloadable versions of the documentation need some time
to get updated, we would like to ask you to be a bit patient.
