The install.txt file for the windows binary installation mentions copying the php4ts.dll to the %SystemRoot% for Apache and Netscape servers. It seems to be required for IIS, if you're using the php4isapi.dll. This copy step needs to be added to the ISAPI installation process.

This is already mentioned in the general install-instructions of the manual, and will be stated in the next PHP-Version
