On Linux I installed PDFlib 3.0 and php 4.0.1pl2.
I got a compilation failure during make of php.
The configuration of php was:
--with-pdflib [defaulted to /usr/local/lib]
the problem is in the function
pdf_get_parameter();
where is defined
char *value;
vlue is used thereafter without asterisk!
I bypassed the problem corecting pdf.c in ~/ext/pdf.c
at line 1877 and 1879.
I think this is quite sufficient for you.

This problem was not present in php 4.0RC2
