I followed the instructions to install PHP4 as an ISAPI for IIS 5, which worked fine.
However I've updated Office 2000 to SR1a, I've updated MS SQL Server 7 to SP2, and PHP4 no longer works at all now.
So I uninstalled IIS, removed PHP4, and reinstalled IIS, and reinstalled PHP4. Observed same problem: PHP files do not execute, and the files are displayed as text. Observed that ASP files still work without a hitch.

I'm notifying this as possible incompatibility with Office 2000 SR1a.

If you see PHP files displayed as text, it means PHP isn't even being executed by IIS. Your configuration information may have been reset in some way; Make sure that IIS has .php files set to be executed by the PHP extension DLL. Note that if IIS crashes, it will often not properly save the updated configuration information. Try to readd the .php mapping, stop IIS (net stop iisadmin), and then restart it.
PHP 4.0 is known to work fine alongside Office 2000 SR1, so that's not the issue.

I've tried entering in the ISAPI in ISAPI Filters and Home Directory, Configuration, but still it does not run PHP.
The ISAPI Filters shows PHP with a green up arrow meaning that it's loaded. And I've also tried using a .php4 extension instead of .php in the Home Directory, Configuration, but still it doesn't run and just shows it as text without parsing.
I've also tried stopping iisadmin and starting w3svc,
but has no success.

Does 4.0.1pl2 still have this problem?

I've decided to close this issue.
I'm noting that the ISAPI filter is more stable on the Windows 2000 Server platform than on Win2000 Professional.
