The pdf installation instructions say:
Under ISAPI Filters,
add a new ISAPI filter. Use PHP as the filter name, and supply a path to the php4isapi.dll.

php4isapi.dll fails to load when IIS is restarted.

Note that it does load for php 4.01 but fails when switching to php 4.04.

you need to restart from command line.

net stop iisadmin

net start w3svc

this reloads it.
