From watching the php irc channel and mailing list the most common problem people have when using the header() function is that they set the header after sending content.

The documentation does address this issue however it's buried so that it's far harder to find than it should be.  It currently sits between an example for caching and an example for content type.

I would recomend moving the paragraph and example to the top of the documentation.  Just above the first note: looks good.

This bug has been fixed in the documentation's XML sources. Since the
online and downloadable versions of the documentation need some time
to get updated, we would like to ask you to be a bit patient.
