If the log_verbosity_level set to INFO (3) or higher it should output the filename of the script when it is cached. In the logs the filename is an empty string.  

Test script:

--- php.ini ---
opcache.enable=1
opcache.enable_cli=1
opcache.log_verbosity_level=4

--- script ---
<?php
echo "Hello World!\n";
?>

Expected result:
