When using acronyms in the XML sources, the HTML output will have duplicate <acronym> tags. This is confirmed using PhD 0.9.1.

Reproduce code:
---------------
(XML source): <acronym>SAPI</acronym>

Expected result:
----------------
Something like this:

<acronym title="Server Application Programming Interface">SAPI</acronym>

Actual result:
--------------
<acronym class="acronym"><acronym title="Server Application Programming Interface">SAPI</acronym></acronym>

- The acronym tag was been rendered by both Generic and PHP Packages, now only PHP
